According to the Ministry of Health’s presentation, the burden of national health insurance (JKN) aka BPJS Health has always been greater than contribution income, at least since 2014.

Deficits continue to occur, except in 2019 and throughout the Covid pandemic.
Therefore, Budi encouraged discussions on increasing BPJS Health contributions.
“This shows that the dynamics of increasing contributions, even though it is politically sensitive, this must be studied continuously to maintain the sustainability of BPJS in providing health services to our community,” encouraged Budi.
“It is our collective duty to explain that BPJS contributions are actually very, very cheap and beneficial for people’s health,” he added.
Budi then dissected the steps in maintaining the financial sustainability of BPJS Health, apart from increasing participant fees.
He emphasized the importance of an efficient contribution system and mechanism.
He gave the example that there will be a Standard Inpatient Class (KRIS) in the implementation of the latest BPJS scheme.According to him, BPJS should be focused on lower class people.
“Even though this is an ongoing debate with BPJS, I say, there’s no need for BPJS

the rich ones.Because the rich are class I, let it be taken privately.”The essence of the KRIS principle is to ensure that the private sector is good,” said Budi.
The number of recipients of BPJS Health contribution assistance (PBI) reached 96.8 million people or 34 percent of the total population of Indonesia.This data was captured by the Ministry of Health as of July 2025.
The Ministry of Health noted that there were still 10.84 million recipients of aid who were not on target, namely from decile 6 to decile 10.
Updating PBI membership by referring to the National Socioeconomic Single Data (DTSEN) is also being encouraged by the Ministry of Health so that aid recipients are only decile 1-decile 5.
On the other hand, Budi emphasized the importance of efficiency in the patient referral flow.He emphasized that in the future the patient referral flow must be competency-based in order to save BPJS expenses.
“Now people who have heart attacks have to have surgery. They come from the Community Health Center, go to a type C hospital first, refer again to type B, then refer again to type A. In fact, the person who can do (surgery) is clearly type A, type C (and) type B cannot possibly be handled. In this way, BPJS should not have to spend money 3 times, just raise it once to the top,” he explained.
